Independent review
Also called fresh-context review, writer/reviewer pattern
Having a separate AI instance, one that didn't write the work and doesn't share its reasoning, review it.
An AI that has just written something still holds every reason it had for writing it that way. Ask it to check its own work and it rereads the work through those same reasons, so a wrong assumption looks as sound the second time as the first.
Independent review breaks that. A second instance gets the work and the criteria, and nothing about how the work was produced. With no reasoning of its own to defend, it judges what's in front of it, which is why it catches subtle problems a self-review tends to miss.
For example
One Claude session writes a currency module; a second session, given only the diff and the criteria, reviews it before it merges.
